Physical Therapy Invoice Template

The physical therapy invoice is given to patients after receiving treatment from a physical therapist (a.k.a., physiotherapist). The invoice states the type of treatment administered to the patient and the costs corresponding with each service. Generally, the invoice is produced and paid upon the completion of each session. If the patient receives multiple treatments over several visits, the invoice should provide a description of each session along with the charges applicable to those dates.
